What Hospice Care Provides

Hospice brings an interdisciplinary team together around the needs, goals and comfort of the patient and family. Depending on the individualized plan of care, services may include nursing care, physician oversight, hospice aide services, social work, spiritual support when desired, counseling, plan-related medications, equipment and supplies, therapy when included in the plan, caregiver support, respite care when appropriate and bereavement support.

Understanding Medicare Hospice Coverage

For eligible Medicare beneficiaries, hospice is covered under the Medicare Hospice Benefit. Eligibility includes certification by the appropriate physicians that the individual is terminally ill with a medical prognosis of six months or less if the illness runs its normal course, along with election of the hospice benefit.

When Should I Call Hospice?

Families often wonder whether it is too early to ask about hospice. You do not have to make that decision alone.

  • More frequent hospitalizations or emergency-room visits
  • Increasing weakness or sleeping
  • Significant decline in eating or drinking
  • Increasing help needed with everyday activities
  • Symptoms becoming more difficult to manage
  • A physician beginning to discuss comfort-focused care

Care referrals shouldn’t create more work.

One call. We’ll take it from here. Embrace coordinates hospice referrals, palliative care referrals, and not-sure care consultations with the referring team, patient and family as appropriate. Clinical eligibility, care planning, and admission decisions remain individualized.

Hospice is a model of care, not only a place.

Hospice may be provided at home, in assisted living, in skilled nursing, or inpatient when clinically appropriate. Palliative care and hospice serve different circumstances; a conversation can help identify the appropriate next step.
